Step 3: Fill in the Required Details

Enter your name, address, Aadhaar number, and other requested personal information.

Step 4: Verify Your Mobile Number

Enter your mobile number and verify it by providing the OTP (One-Time Password) you receive.

Step 5: Select Your Health Centre
உங்கள் மாவட்டம், வட்டாரம் மற்றும் உங்கள் பகுதிக்கு அருகிலுள்ள ஆரம்ப சுகாதார நிலையம் / துணை சுகாதார மையத்தைத் தேர்ந்தெடுக்கவும்.
Choose your District, Block, and the Primary Health Centre / Health Sub-Centre nearest to your location.

Step 6: Choose an Appointment Date
கிராம சுகாதார செவிலியர் (VHN) உங்கள் வீட்டிற்கு வந்து சரிபார்ப்பதற்காக ஒரு வசதியான தேதியைத் தேர்ந்தெடுக்கவும்.
Select a convenient date for the Village Health Nurse (VHN) to visit your home for verification.
படி 7: படிவத்தைச் சமர்ப்பிக்கவும்
Step 7: Submit the Form
அனைத்து விவரங்களையும் சரிபார்த்து, படிவத்தைச் சமர்ப்பிக்கவும். எதிர்கால பயன்பாட்டிற்காக உருவாக்கப்படும் ஒப்புகை எண்ணை (acknowledgment number) குறித்துக் கொள்ளவும்.
Review all the details and submit the form. Note down the acknowledgment number generated for future reference.
